Painkiller returns with brutal co-op shooter action this fall

3D Realms is bringing back the blood-soaked chaos of Painkiller with a modern twist, and it’s looking seriously gnarly. Revealed during the Future Games Show, this reimagining of the cult FPS is set to launch in fall 2025 on PlayStation 5, Xbox Series X|S, and PC via Steam.

The new Painkiller dials everything up: intense combat, a metal-fueled soundtrack, wild weapons (both old favourites and new creations), and plenty of demons to mow down. Whether you’re playing solo or with up to two friends in online co-op, this is shaping up to be a visceral blast of classic shooter action.

The setup? You’re stuck in Purgatory, punished for your sins. But all is not lost: the Creator’s Voice offers you one shot at redemption. That means blasting your way through gothic hellscapes, taking down demonic hordes and nightmarish bosses, and trying to stop the fallen angel Azazel from invading Earth.

Gameplay-wise, it looks like a frantic mix of classic FPS and modern movement. You can jump, dash, and hook your way across bloody battlefields, laying waste to everything in sight. Alongside weapons and tarot-card upgrades, you’ll get to choose from four unique characters, each with different perks: Ink, Void, Sol, and Roch.

Expect nightmarish enemies (including Azazel’s monstrous Nephilim children), gothic environments, secrets to uncover, and plenty of ways to dish out punishment. Whether solo with bots or online with pals, Painkiller is all about fast, gory, adrenaline-fueled action.

Painkiller launches this fall on PS5, Xbox Series X|S and PC.
